We are seeking a dedicated and compassionate Adult Social Worker to join our dynamic team at a leading hospital in Norfolk for a 6-month contract. This role is crucial in providing social work support to adults within a hospital setting, ensuring they receive the care and resources they need during their recovery. The ideal candidate will have a strong commitment to improving patient outcomes and a passion for helping individuals navigate complex social and health-related challenges.

Key responsibilities include:

  • Conducting assessments of patients' social and emotional needs upon admission.
  • Developing and implementing care plans in collaboration with multidisciplinary teams.
  • Providing counseling and support to patients and their families, addressing emotional and practical concerns.
  • Facilitating discharge planning, ensuring a smooth transition to home or alternative care settings.
  • Advocating for patients’ rights and accessing community resources and services.

To be considered for this position, candidates must possess the following essential qualifications and skills:

  • A recognized degree in Social Work and current registration with Social Work England.
  • Experience working in a hospital or healthcare environment, preferably with adult clients.
  • Strong assessment and intervention skills, with the ability to work under pressure.
  • Excellent communication and interpersonal skills, with a compassionate approach to patient care.
  • Knowledge of relevant legislation and policies affecting adult social care.
