Location: South London

Contract: Permanent, Full Time (36 hours per week)

Salary: £42,060-£50,085 per annum (up to £52,194 unconsolidated maximum)

Bonus: Welcome Bonus of £1,000 (paid in month one) or an annual Retention Bonus of £1,000 (both subject to payscale)

About the role

The Adult Social Care Locality Team help adults live as independently as possible. You will work within integrated Locality teams aligned to Primary Care Networks, collaborating closely with NHS colleagues and wider partners to deliver holistic, person-centred support.

What you will do:

  • Undertake Care Act 2014 assessments, ensuring personalised, outcomes-focused planning.
  • Complete Care Act reviews to ensure care and support plans remain proportionate, effective and strengths-based.
  • Lead safeguarding enquiries, addressing risk and promoting safety and wellbeing.
  • Carry out Mental Capacity assessments and contribute to best-interest decision-making.
  • Manage duty on a rota basis and contribute to DSTS and CHC activity.
  • Prepare high-quality reports and undertake court-related work where required.
  • Work collaboratively with professionals, families/carers and external agencies to co-produce outstanding care plans.

About you:

  • Registered with Social Work England and qualified in Social Work (or equivalent).
  • At least 2 years' post-ASYE UK experience within adult services.
  • Confident working in multidisciplinary settings with complex mental and physical health needs.
  • Skilled at communicating complex information to residents and professionals.
  • Strong organisational skills, able to coordinate social care plans and support safe, timely discharges.
  • Compassionate, resilient and motivated to deliver positive outcomes under pressure.
  • Willing to travel outside the Borough to visit residents when required.
