In this role, you will provide essential support and interventions, working collaboratively with clients, their families, and other professionals to promote recovery and well-being.
Your key responsibilities will include:
- Conducting comprehensive assessments of clients' needs and developing tailored care plans.
- Providing direct support and guidance to clients, helping them navigate their recovery journey.
- Collaborating with multidisciplinary teams to ensure holistic care and effective service delivery.
- Advocating for clients’ rights and access to appropriate resources and services.
- Monitoring and reviewing clients’ progress and adapting interventions as necessary.
The ideal candidate will possess essential qualifications and skills such as:
- A recognized social work qualification (Degree in Social Work or equivalent).
- Current registration with Social Work England.
- Experience working within mental health and substance misuse services.
- Strong assessment and intervention skills, with a client-centered approach.
- Excellent communication and interpersonal skills, with the ability to build rapport with diverse populations.
