We are seeking a dedicated and experienced Qualified Social Worker to join our Integrated Reviewing Team in Leeds, focusing on adult services. This role is pivotal in ensuring that service users receive the highest standard of care and support. You will work collaboratively with various stakeholders to assess, review, and monitor the needs of adults, ensuring their voices are heard and their rights are upheld.
Key responsibilities include:
- Conducting comprehensive assessments and reviews of adult service users to identify their needs and outcomes.
- Developing and implementing person-centered care plans in collaboration with service users and their families.
- Working closely with multidisciplinary teams to coordinate services and support for individuals.
- Monitoring and evaluating the effectiveness of care plans and making necessary adjustments.
- Ensuring compliance with relevant legislation and safeguarding policies.
To be considered for this role, you must possess the following essential qualifications and skills:
- A recognized social work qualification (e.g., BA/MA in Social Work).
- Registration with Social Work England.
- Proven experience in adult social work, particularly in assessment and review processes.
- Strong communication and interpersonal skills, with the ability to engage effectively with diverse individuals.
- Excellent organizational and time management abilities.
