In this role, you will be responsible for conducting annual reviews for individuals receiving support, ensuring their needs are met and their rights are upheld. Key responsibilities include:
- Conducting comprehensive assessments and annual reviews for service users.
- Developing and implementing care plans in collaboration with service users and their families.
- Monitoring and evaluating the effectiveness of services provided to ensure optimal support.
- Providing advice, support, and guidance to individuals, families, and other professionals.
- Maintaining accurate and timely records in compliance with legislation and policies.
To be successful in this role, you will need the following essential qualifications and skills:
- A recognized social work qualification (e.g., BA/MA in Social Work).
- Registration with Social Work England.
- Experience in adult social care or similar settings.
- Strong communication and interpersonal skills.
- Ability to work independently and as part of a multidisciplinary team.
