You will be responsible for providing high-quality social work support, ensuring the well-being and development of our clients while working collaboratively with families and other professionals.
Your key responsibilities will include
- Conducting assessments to determine the needs of clients and their families.
- Developing and implementing care plans tailored to individual client needs.
- Providing ongoing support and guidance to clients and their families.
- Working collaboratively with multi-disciplinary teams to ensure comprehensive service delivery.
- Maintaining accurate and up-to-date case records in compliance with legal and organizational standards.
To be successful in this role, you will need to possess the following essential qualifications and skills:
- A recognised social work qualification (e. g., BA/MA in Social Work).
- Registration with Social Work England.
- Experience working with individuals with learning disabilities and autism.
- Strong communication and interpersonal skills.
- Ability to work independently and as part of a team.
