Your key responsibilities will include:
- Conducting comprehensive assessments of clients’ needs and circumstances.
- Developing and implementing personalized care plans in collaboration with clients and their families.
- Monitoring and reviewing care plans to ensure effectiveness and compliance with legal and regulatory frameworks.
- Providing information, advice, and support to clients, their families, and other professionals.
- Maintaining accurate and up-to-date case records and documentation.
- Participating in multi-disciplinary meetings and contributing to the safeguarding of vulnerable adults.
To be considered for this role, you must possess a relevant social work qualification (e.g., BA/MA in Social Work) and be registered with Social Work England. We require strong interpersonal skills, the ability to work effectively in a team, and a commitment to continuous professional development. Experience in adult social care and knowledge of relevant legislation and policies are essential.
