Dorchester, Dorset | Hybrid Working

Up to £38 per hour Umbrella | £150 Accommodation Allowance

Are you confident working across both Children's and Adults Social Care? This is a fantastic opportunity for an experienced Social Worker to join Dorset's Birth to Settled Adulthood (B2SA) Service, supporting children and young people aged 0-25 as they prepare for adulthood. Sitting across the traditional divide between Children's and Adults Services, this role would particularly suit someone with experience in Children with Disabilities, Learning Disabilities, SEND, Transitions or Preparing for Adulthood. You'll work with young people with autism, learning disabilities, SEND and complex needs, including those at risk of mental health hospital admission.

What you'll be doing:

  • Managing a caseload of children and young people on the Dynamic Support Register (DSR)
  • Acting as a key point of contact for young people, families and carers
  • Supporting young people through Preparing for Adulthood and transition
  • Developing and coordinating person-centred support plans
  • Working closely with Occupational Therapists, NHS teams, education and other social care professionals
  • Contributing to Care, Education and Treatment Reviews (CETRs)
  • Supporting hospital admission avoidance and discharge planning
  • Completing assessments, care planning and risk management
  • Providing updates at DSR meetings and multi-agency forums
  • Ensuring joined-up support across Children's and Adults Services

Who are we looking for?

You could come from either Children's or Adults Social Care, but you'll need to be comfortable working across both disciplines and supporting young people through transition. You'll need:

  • Social Work England registration
  • Minimum 2 years' post-qualified experience
  • Experience within Children's and/or Adults Social Care
  • Confidence working with young people aged 0-25
  • Strong assessment, care planning and risk management skills
  • Good knowledge of relevant legislation, including the Children Act, Care Act and Mental Capacity Act
  • Experience of multi-agency working across health, education and social care
  • Full UK driving licence and access to a vehicle

Particularly advantageous experience:

  • Children with Disabilities Social Work
  • Learning Disabilities
  • Preparing for Adulthood / Transitions
  • Working alongside Occupational Therapists
  • Autism, SEND and complex needs

If you're an experienced CWD Social Worker who understands the journey into Adult Services, or an Adults/LD Social Worker who is confident working with younger people and families, I'd be particularly interested in speaking with you.

Why consider this role?

  • Up to £38 per hour Umbrella
  • £150 accommodation allowance
  • Hybrid working
  • Unique 0-25 service, spanning Children's and Adults Social Care
  • Specialist work across Learning Disabilities, Autism, SEND and complex needs
  • Strong multi-disciplinary working with OTs, health, education and social care
  • Excellent opportunity to develop specialist Preparing for Adulthood/Transitions experience

If you're confident working across Children's and Adults Services and want a role where you can support young people through one of the most important transitions in their lives, I'd love to hear from you.
