We are seeking a dedicated and compassionate Qualified Social Worker to join our Adult's Learning Disabilities (LD) Team in Darlington. This role is pivotal in supporting adults with learning disabilities to achieve their full potential and lead fulfilling lives. As a key member of our team, you will work collaboratively with individuals, families, and other professionals to assess needs, develop care plans, and ensure the delivery of high-quality support services.
Your key responsibilities will include:
- Conducting comprehensive assessments of individuals with learning disabilities to identify their needs and preferences.
- Developing, implementing, and reviewing person-centered care plans in collaboration with individuals and their families.
- Providing ongoing support and guidance to individuals, ensuring they have access to appropriate resources and services.
- Working closely with multidisciplinary teams to ensure a holistic approach to care and support.
- Maintaining accurate and up-to-date records in compliance with relevant legislation and best practices.
To be successful in this role, candidates must possess the following essential qualifications and skills:
- A recognized social work qualification (e.g., Degree in Social Work, DipSW, or equivalent).
- Registered with Social Work England.
- Experience working with adults with learning disabilities or within a similar field.
- Strong communication and interpersonal skills, with the ability to build rapport and trust with clients.
