Children's SW - Leaving Care Team - Hackney

Join our dedicated Leaving Care Team in Hackney as a Qualified Social Worker specializing in Children's Services. We are seeking a passionate and committed individual who will make a positive impact on the lives of young people transitioning from care to independent living. In this role, you will work closely with young people aged 16-25, providing support, guidance, and advocacy to ensure their successful transition into adulthood.

Your key responsibilities will include:

• Conducting comprehensive assessments of young people's needs and developing tailored support plans.
• Building trusting relationships with young people, empowering them to express their views and make informed decisions.
• Coordinating and facilitating access to housing, education, employment, and health services.
• Maintaining accurate and up-to-date case records and documentation in compliance with statutory requirements.
• Collaborating with multi-agency partners to ensure holistic support for young people.

To be considered for this role, you must possess:

• A recognized social work qualification (BA/MA in Social Work, DipSW, or equivalent).
• Registration with Social Work England.
• Experience working with young people, particularly in a leaving care or related context.
• Strong assessment and analytical skills, with the ability to manage complex cases effectively.
• Excellent communication and interpersonal skills, with a commitment to promoting the rights and interests of young people.
