In this position, you will lead case management efforts, ensuring high-quality support and safeguarding for individuals with complex needs.
Your expertise will be crucial in assessing situations, developing care plans, and collaborating with multidisciplinary teams to deliver effective interventions.
Your key responsibilities will include
- Conducting comprehensive assessments of clients' needs and risks.
- Developing and implementing person-centered care plans in collaboration with clients and their families.
