Your key responsibilities will include:

  • Developing, implementing, and reviewing care plans in collaboration with service users and their families.
  • Working alongside multi-disciplinary teams to coordinate services and support.
  • Maintaining accurate and timely case records and documentation.
  • Providing guidance and support to less experienced team members.

To be considered for this role, you must possess the following essential qualifications and skills:

  • A recognized social work qualification (e.g., Degree in Social Work, CQSW, DipSW).
  • Current registration with Social Work England.
  • Proven experience in adult social work, particularly within long-term support environments.
  • Strong assessment and analytical skills with a person-centered approach.
  • Excellent communication and interpersonal skills, with the ability to build rapport with diverse individuals.
