About the Role
This is a fantastic opportunity to work within a specialist Transitions Team, supporting young people with disabilities as they prepare for adulthood, while also liaising closely with adult services. You'll play a key role in ensuring smooth, well‑planned transitions by delivering high‑quality assessments and safeguarding support at a critical stage in service users' lives.
What You'll Be Doing
- Completing strengths-based assessments across both children's and adult services
- Supporting young people through the Preparing for Adulthood pathway
- Managing a caseload involving adult disability and transition cases
- Undertaking safeguarding work in line with statutory frameworks
- Working collaboratively with families, carers, and multi‑agency partners
- Ensuring timely and effective care planning and reviews
What We're Looking For
- Are a Qualified Social Worker (Social Work England registered)
- Have experience across transitions, adult disability or children's services
- Are confident in completing assessments and managing caseloads independently
- Have strong safeguarding experience
- Can hit the ground running and adapt quickly in a fast‑paced team
