We are seeking a compassionate and dedicated Social Worker to join our Adults Community team in the East Riding. In this rewarding role, you will work collaboratively with individuals and their families to promote well-being and independence, ensuring that vulnerable adults receive the support they need. You will be responsible for assessing client needs, developing care plans, and connecting individuals with appropriate resources and services.
Your key responsibilities will include:
- Conducting comprehensive assessments of adults in need of social care services.
- Developing, implementing, and reviewing personalized care plans in collaboration with clients and their families.
- Providing ongoing support and guidance to clients, ensuring their voices are heard and respected.
- Working closely with other professionals and agencies to coordinate services and improve client outcomes.
- Maintaining accurate and up-to-date case records and documentation.
The ideal candidate will possess the following qualifications and skills:
- A recognized Social Work qualification (Degree in Social Work or equivalent).
- Registration with Social Work England.
- Experience in adult social care and knowledge of relevant legislation and best practices.
- Strong communication and interpersonal skills, with the ability to build rapport with clients and stakeholders.
- Excellent organizational skills and the ability to manage a varied caseload effectively.
