The East Riding of Yorkshire Council is seeking a dedicated and experienced Adult's Social Worker to join our Assessment Team. This pivotal role involves working collaboratively to assess and support adults in need of care and support services. You will play a crucial role in ensuring that the needs of vulnerable individuals are met, promoting their independence and well-being within the community.
Key responsibilities include:
- Conducting thorough assessments and reviews of adult service users to determine their needs and eligibility for services.
- Developing and implementing personalized care plans in collaboration with service users and their families.
- Working closely with multidisciplinary teams and external agencies to ensure integrated support and safeguarding.
- Maintaining accurate records and documentation in line with statutory requirements and best practice standards.
- Providing advice and support to service users, advocating on their behalf to ensure they receive appropriate services.
To be considered for this position, candidates must possess the following qualifications and skills:
- A relevant social work qualification (e.g., Degree in Social Work, DipSW, or equivalent).
- Registration with Social Work England.
- Proven experience in adult social work, particularly in assessment and care planning.
- Strong communication and interpersonal skills, with the ability to build rapport with diverse individuals.
- Excellent organizational skills and the ability to manage a caseload effectively.
