We are seeking a dedicated and compassionate Qualified Social Worker to join our Adult's Social Work Locality Team in Bath and North East Somerset. Key responsibilities and duties include:

• Conducting comprehensive assessments of individuals’ needs and risks.
• Developing, implementing, and reviewing person-centered care plans.
• Collaborating with multidisciplinary teams and external agencies to ensure holistic support.
• Providing ongoing support and guidance to service users and their families.
• Maintaining accurate and up-to-date case records and documentation.
• Participating in supervision and professional development opportunities.

To be considered for this role, you must possess the following essential qualifications and skills:

• A recognized social work qualification (e.g., BA/MA in Social Work).
• Current registration with Social Work England.
• Proven experience in adult social work, preferably within a locality team.
• Strong assessment and analytical skills.
• Excellent communication and interpersonal skills.
• Ability to work independently and as part of a team.
