We are seeking a dedicated and compassionate Social Worker to join our Adults Community Team in Devon. In this rewarding role, you will work closely with adults and their families, providing essential support and guidance to enhance their well-being and independence. Your expertise will be vital in assessing needs, developing care plans, and coordinating services to ensure that individuals receive the best possible care tailored to their unique circumstances.

Key responsibilities include:

  • Conducting comprehensive assessments of individuals' needs and risks.
  • Developing and implementing personalized care plans in collaboration with clients and their families.
  • Coordinating with multidisciplinary teams and external agencies to facilitate access to services.
  • Providing ongoing support, monitoring progress, and adjusting care plans as needed.
  • Advocating for clients' rights and ensuring they are informed about their options and resources.

The ideal candidate will possess the following qualifications and skills:

  • A recognized degree in Social Work and registration with Social Work England.
  • Proven experience in adult social work, particularly in a community setting.
  • Strong assessment and analytical skills, with the ability to make sound decisions under pressure.
  • Excellent communication and interpersonal skills, with a compassionate approach to client care.
  • Ability to work collaboratively within a team and engage effectively with other professionals.
