Join our dynamic Assessment Team in Devon as a Qualified Social Worker specializing in Adult Services. This is a fantastic opportunity to make a real difference in the lives of vulnerable adults by conducting comprehensive assessments, developing care plans, and ensuring the delivery of high-quality support services. You will work collaboratively with individuals, their families, and other professionals to identify needs and promote well-being.

Your key responsibilities will include:

  • Conducting thorough assessments of adults in need of care and support.
  • Developing and implementing person-centered care plans in collaboration with service users.
  • Monitoring and reviewing care plans to ensure they meet the evolving needs of individuals.
  • Providing advice and support to families and caregivers.
  • Maintaining accurate and up-to-date case records in compliance with statutory requirements.

To be successful in this role, you must possess the following essential qualifications and skills:

  • A recognized social work qualification (e.g., Degree in Social Work, DipSW, or equivalent).
  • Registration with Social Work England.
  • Experience in adult social work, particularly in assessment and care planning.
  • Strong communication and interpersonal skills.
  • Ability to work independently and as part of a multidisciplinary team.
