Must have MH experience and Minimum of 2 years post qualification experience * Work collaboratively across a range of disciplines to continuously promote a ‘needs based’ approach to all service users. * Promote a partnership approach to service delivery. * Develop clear support plans following a Care Act assessment, Complete funding applications, complete work that has legal requirements such as Copdol, dols, 21a’s, reports for court etc * Develop meaningful engagement with individuals who are often referred to as ‘difficult to engage’ and their families. * Support Carers through the Care Act * Work with Children about to turn 18 and transition into Adult Mental Health services * The team will offer a 6-day service within the hours of 09.00hrs to 17.00hrs, the majority being Monday to Friday with the occasional Saturdays.
