We are seeking a dedicated and compassionate Qualified Social Worker to join our Adult's Social Work - Physical Disabilities (PD) Team in Southwark. As a Qualified Social Worker in the PD Team, your key responsibilities will include:

• Assessing the needs and circumstances of adults with physical disabilities to develop tailored support plans.
• Providing ongoing support and guidance to clients and their families, ensuring they have access to necessary resources and services.
• Working collaboratively with healthcare professionals, community organizations, and other stakeholders to promote holistic care.
• Conducting regular reviews of care plans and adjusting them as needed to reflect changes in client circumstances.
• Maintaining accurate and up-to-date case records and documentation in compliance with organizational policies and procedures.

The ideal candidate will possess the following essential qualifications and skills:

• A recognized social work qualification (e.g., CQSW, DipSW, or equivalent).
• Registration with the Health and Care Professions Council (HCPC).
• Proven experience in adult social work, particularly with individuals with physical disabilities.
• Strong assessment and analytical skills, with the ability to formulate effective care plans.
• Excellent communication and interpersonal skills, with a commitment to client-centered practice.
