JOB TITLE: Adult Social Worker – Hayes & Five Elms Locality Team We are looking for a social worker to work within Hayes and Five Elms Locality Team. The Locality teamwork is an ongoing care management intervention, and the coordination team leads on annual reviews of support plans, extra care housing support and direct payment work. This service also provides assessments under Care Act 2014 and work with young people transitioning from Children's services to Adult services, and beyond. You will be supporting Younger Adults with physical disabilities and sensory impairment and Older People to live as independently as they are able and work with their carers to ensure risks are appropriately managed. You will need to be flexible, committed, and hard working with a willingness to work to quality standards. You will be working within a multidisciplinary framework and need to have good communication and negotiating skills.
