Join our dedicated Adult's Social Work - Learning Disability Team in Tower Hamlets, where you will play a vital role in enhancing the lives of adults with learning disabilities. As a Qualified Social Worker, you will be responsible for providing holistic assessments, developing tailored care plans, and ensuring the delivery of high-quality support services. Your expertise will help empower individuals to achieve their personal goals and enhance their independence within the community.

Your key responsibilities will include:

  • Conducting comprehensive assessments of individuals with learning disabilities to identify their needs and strengths.
  • Developing, implementing, and reviewing person-centered care plans in collaboration with service users and their families.
  • Working closely with multi-disciplinary teams, external agencies, and community resources to facilitate effective support.
  • Providing guidance and support to families and carers, ensuring they are well-informed and involved in the care process.
  • Maintaining accurate and up-to-date case records in compliance with local and national policies.

The ideal candidate will possess the following essential qualifications and skills:

  • A recognized Social Work qualification (e.g., Degree in Social Work, DipSW, or equivalent).
  • Current registration with Social Work England.
  • Proven experience working with adults with learning disabilities, preferably within a social work setting.
  • Strong communication and interpersonal skills, with the ability to build rapport with service users and stakeholders.
  • Excellent organizational skills and the ability to manage a diverse caseload effectively.
