We are seeking a dedicated and compassionate Qualified Social Worker to join our Adult's Social Work Hospital Team in the London Borough of Lambeth. You will play a crucial role in assessing and addressing the needs of individuals, ensuring they receive the appropriate support and resources during their hospital stay and upon discharge.
Key responsibilities and duties include:
- Conducting comprehensive assessments of individuals’ needs and risks in collaboration with healthcare professionals.
- Developing and implementing person-centered care plans that promote independence and well-being.
- Providing information, advice, and support to individuals and their families regarding available services and resources.
- Facilitating safe discharges from hospital, coordinating with community services and other stakeholders.
- Maintaining accurate and up-to-date case records in line with statutory requirements and organizational policies.
The ideal candidate will possess the following essential qualifications and skills:
- A degree in Social Work and registration with Social Work England.
- Proven experience in adult social work, particularly in a hospital or health care setting.
- Strong assessment and analytical skills, with the ability to make informed decisions under pressure.
- Excellent communication and interpersonal skills, with the ability to build rapport with diverse individuals.
- Knowledge of relevant legislation and best practices in social care.
