We are seeking a dedicated and passionate Qualified Social Worker to join our Adult's Social Work Hospital Team in Hackney.
This role offers the opportunity to work in a dynamic environment, supporting adults with complex needs and ensuring they receive the care and support essential for their well-being.
You will collaborate with healthcare professionals, families, and community organizations to facilitate seamless transitions from hospital to home and promote independence.
Key responsibilities include
- Conducting assessments to identify the needs of adults and their families.
- Developing and implementing care plans in partnership with service users.
- Providing emotional and practical support to individuals and their families.
- Collaborating with multidisciplinary teams to ensure holistic care delivery.

To be successful in this role, candidates must possess the following qualifications and skills:
