This Post is within Mid Notts Discharge to Assess Service. The role will predominantly be undertaking social care assessments with people ether at home or whilst in an assessment bed to arrange the longer term social care support that they will require. There will be a need to complete care act assessments using the three conversations approach, mental capacity act assessments as required , involvement in CHC assessments and undertaking the safeguarding officer role for S42 enquiries.
