London Borough of Merton is seeking a dedicated and passionate Qualified Social Worker to join our Adult's Social Work - Community/LD Team. This role is crucial in supporting individuals with learning disabilities and other complex needs, ensuring they receive the best possible care and support to lead fulfilling lives. As a member of our dynamic team, you will play a key role in promoting independence and empowering clients within the community.

Key Responsibilities

  • Assessing the needs of adults with learning disabilities and developing tailored support plans.
  • Working collaboratively with clients, families, and multi-disciplinary teams to ensure effective service delivery.
  • Monitoring and reviewing care plans to adapt to changing needs and circumstances.
  • Providing guidance and support to families, advocating for clients' rights and choices.
  • Maintaining accurate and up-to-date case records in compliance with relevant legislation.

Qualifications and Skills

  • A recognized social work qualification (e.g., BA/MA in Social Work).
  • Registration with Social Work England.
  • Experience working within adult social care, particularly with individuals with learning disabilities.
  • Strong assessment and analytical skills, with the ability to manage complex cases.
  • Excellent communication and interpersonal skills, with a commitment to partnership working.
