Are you an experienced Qualified Social Worker looking to make a real impact supporting older people living with dementia? I am working with a well-established Adult Social Care team in Newcastle who are looking for someone who can step straight into a busy, rewarding role supporting vulnerable adults and their families.
The Role
You will be working with older adults affected by dementia, completing person-centred assessments, developing care plans, and working closely with families and partner agencies to deliver the right support at the right time. You will be part of a multi-disciplinary team focused on maintaining dignity, independence and quality of life for service users.
Essential Requirements (Non-Negotiable)
- Qualified Social Worker (Social Work England registered)
- Proven experience working as a Social Worker (Adults)
- Strong background in assessment, care planning and safeguarding
- Experience working with older people and/or dementia services
This is not a role for someone stepping up for the first time - you must be confident managing a caseload and making informed decisions.

- Carry a caseload of adults with complex needs, including dementia
- Undertake strengths-based and person-centred assessments
- Develop and review care plans in line with the Care Act
