In this pivotal role, you will work collaboratively with families, children, and various stakeholders to ensure the best outcomes for those in our care.
Your key responsibilities will include:
- Conducting assessments and developing care plans tailored to the individual needs of children and families.
- Building strong relationships with children and young people to understand their needs and aspirations.
- Working closely with multi-disciplinary teams to provide integrated support and intervention.
- Monitoring and reviewing the progress of care plans, ensuring compliance with statutory requirements.
- Providing guidance and support to foster carers and adoptive families.
To be considered for this role, you must possess a relevant social work qualification (e.g., Degree in Social Work or equivalent) and be registered with Social Care Wales. We are looking for candidates who demonstrate:
