Your expertise will be crucial in assessing needs, developing care plans, and advocating for patients to ensure they receive the best possible care during their hospital stay and beyond.
Key responsibilities include:
- Conducting comprehensive assessments to identify patients' social needs and risks.
- Developing and implementing individualized care plans in collaboration with multidisciplinary teams.
- Providing emotional support and counseling to patients and their families.
- Facilitating access to community resources and services post-discharge.
- Maintaining accurate and up-to-date records in accordance with legal and ethical guidelines.
To be considered for this position, you must possess the following qualifications and skills:
- A recognized degree in Social Work and current registration with Social Work England.
- Proven experience in adult social work, preferably in a hospital or healthcare setting.
- Strong assessment, communication, and interpersonal skills.
- Ability to work effectively within a multidisciplinary team and manage a varied caseload.
- Knowledge of relevant legislation and policies affecting adult social care.
