As a key member of our team, you will be responsible for:
- Conducting comprehensive assessments and developing tailored support plans for individuals with learning disabilities.
- Collaborating with multidisciplinary teams, families, and external agencies to ensure cohesive support and care.
- Providing ongoing support and advocacy to service users, empowering them to achieve their personal goals.
- Monitoring and reviewing care plans to ensure they meet the evolving needs of individuals.
- Maintaining accurate records and documentation in compliance with regulatory standards.
The ideal candidate will possess the following qualifications and skills:
- A recognized social work qualification (e.g., BA/MA in Social Work).
- Registration with Social Work England.
- Experience working with adults with learning disabilities in a social work capacity.
- Strong communication and interpersonal skills, with the ability to build rapport with service users and their families.
- Excellent organizational skills and the ability to manage a varied caseload effectively.
