Your key responsibilities will include:
- Conducting assessments of individuals’ needs and determining appropriate interventions.
- Developing and implementing care plans in collaboration with service users and their families.
- Providing ongoing support and guidance to service users, ensuring they have access to relevant resources and services.
- Working collaboratively with multidisciplinary teams to ensure holistic care and support.
- Maintaining accurate and up-to-date case records in compliance with organizational policies and procedures.
- Participating in team meetings and contributing to the development of best practices.
To be successful in this role, you must possess the following essential qualifications and skills:
- A recognized social work qualification (e.g., Degree in Social Work, DipSW, or equivalent).
- Current registration with Social Work England.
- Demonstrable experience working within adult social care, preferably in a locality team setting.
- Strong assessment and analytical skills, with the ability to develop effective care plans.
- Excellent communication and interpersonal skills, with a compassionate approach to service users.
- Knowledge of relevant legislation, policies, and procedures in adult social care.
