We are seeking a dedicated and compassionate Social Worker to join our Adults Transition team in Cornwall. This crucial role involves supporting adults in their transition to independence and ensuring they receive the necessary services and resources to thrive. As a Social Worker, you will work collaboratively with individuals, families, and multidisciplinary teams to promote well-being and enhance quality of life.
Your key responsibilities will include:
- Conducting assessments to identify the needs and strengths of individuals transitioning to adulthood.
- Developing and implementing personalized care plans in collaboration with clients and their families.
- Providing ongoing support and guidance to clients as they navigate services and resources.
- Advocating for clients' rights and ensuring access to appropriate services.
- Maintaining accurate and up-to-date records of client interactions and progress.
- Participating in team meetings and contributing to the development of best practices within the team.
To be successful in this role, you will need the following qualifications and skills:
- A recognized Social Work qualification (Degree in Social Work or equivalent).
- Registered with Social Work England.
- Experience in adult social care, particularly with transition services.
- Strong communication and interpersonal skills, with the ability to build rapport with clients and stakeholders.
- Excellent organizational skills and the ability to manage a varied caseload.
- A commitment to continuous professional development and reflective practice.
