Your role will involve conducting assessments, developing care plans, and collaborating with various stakeholders to promote the wellbeing and independence of adults in need.

Your key responsibilities will include:

  • Conducting comprehensive assessments of adults' needs and risks.
  • Developing and implementing care plans in partnership with service users.

To be successful in this role, you must possess the following qualifications and skills:

  • A recognized social work qualification (e.g., BA/MA in Social Work).
  • Registered with Social Work England.
  • Experience in adult social work, particularly in community settings.
  • Strong assessment and analytical skills.
  • Excellent communication and interpersonal skills.
  • Ability to work collaboratively within a multidisciplinary team.
