Caritas Recruitment is currently recruiting for an experienced Social Worker to join the Preparing for Adulthood Team (PfA) at the London Borough of Bromley.
This rewarding role sits within Bromley's 0-25 Service and focuses on supporting young people aged 17Β½ to 25 with learning disabilities, autism and additional needs as they transition into adulthood.
Key Responsibilities
- Undertake Care Act assessments, reviews and support planning.
- Complete Mental Capacity Act assessments and Best Interests decisions.
- Manage a caseload of young people transitioning into adult services.
- Carry out safeguarding enquiries and risk management activity.
- Work closely with families, health professionals, education providers and partner agencies.
- Participate in Court of Protection and Best Interests work where required.
- Maintain accurate assessments, reports and case records.
About You
- Qualified Social Worker registered with Social Work England.
- Strong Adult Social Care experience.
- Sound knowledge of the Care Act 2014 and Mental Capacity Act 2005.
- Experience undertaking assessments, support planning and safeguarding work.
- Ability to work effectively within a multi-agency environment.
Desirable Experience
- Preparing for Adulthood or transition work.
- Supporting young adults with learning disabilities and/or autism.
- Court of Protection experience.
