We are seeking a dedicated and compassionate Social Worker to join our Leaving Care Team in Sutton. This role is pivotal in supporting young people transitioning from care to independent living. As a Social Worker, you will work closely with young adults aged 16-25, providing them with the guidance, resources, and advocacy they need to thrive and achieve their goals.
Your key responsibilities will include:
- Conducting comprehensive assessments to identify the individual needs of young people leaving care.
- Developing and implementing personalized support plans in collaboration with young adults and relevant stakeholders.
- Providing emotional and practical support to young people, helping them navigate challenges associated with independence.
- Facilitating access to education, employment, and training opportunities.
- Building strong relationships with young people, fostering trust and encouraging their involvement in decision-making processes.
- Maintaining accurate and timely records, ensuring compliance with statutory requirements.
To be successful in this role, you will need the following essential qualifications and skills:
- A recognized Social Work qualification (BA/MA in Social Work or equivalent).
- Registration with Social Work England.
- Experience working with young people in a social care setting, particularly with those in or leaving care.
- Strong communication and interpersonal skills, with the ability to engage effectively with diverse groups.
- Excellent organizational skills and the ability to manage a caseload effectively.
- A commitment to promoting the welfare and rights of young people.
