As a Social Worker within our Learning Disability Team, your key responsibilities will include:

- Developing and implementing person-centered support plans that promote independence and well-being.
- Collaborating with multidisciplinary teams, families, and external agencies to ensure a holistic approach to care.
- Providing ongoing support, guidance, and advocacy for clients and their families.
- Monitoring and reviewing client progress, adjusting support plans as necessary to meet changing needs.
To be considered for this role, candidates must possess the following essential qualifications and skills:
- A recognized Social Work qualification (BA/MA in Social Work or equivalent).
- Registration with Social Work England.
- Experience working with individuals with learning disabilities or within a similar field.
- Strong communication and interpersonal skills, with the ability to build rapport with clients and their families.
- Excellent organizational skills and the ability to manage a varied caseload effectively.
