As a Social Worker in this role, your key responsibilities will include:
- Conducting assessments to identify the needs and strengths of individuals with learning disabilities.

- Facilitating smooth transitions for individuals moving from children’s to adult services.
- Providing ongoing support and guidance to families, ensuring they are informed and engaged in the process.

The ideal candidate will possess the following essential qualifications and skills:
- A recognized Social Work qualification (e.g., Degree in Social Work, DipSW, or equivalent).
- Current Social Work registration with Social Work England.
- Experience working with individuals with learning disabilities and knowledge of relevant legislation.
- Strong communication and interpersonal skills, with the ability to build rapport with diverse populations.
- Excellent organizational skills and the ability to manage a caseload effectively.
