We are seeking a qualified Adult Social Worker to join a busy Locality Team supporting adults, older people and their families to live safely, independently and with greater choice. You will manage a varied caseload, completing strengths-based assessments, reviews and care planning in line with relevant legislation, statutory guidance and professional standards. The role involves working with people who may have complex care and support needs, including physical disabilities, mental health needs, learning disabilities, frailty or risks associated with safeguarding.
You will work collaboratively with individuals, carers, families, health professionals, care providers and community organisations to identify outcomes and coordinate effective support. Responsibilities will include undertaking proportionate assessments, responding to safeguarding concerns, arranging and reviewing services, promoting independence and wellbeing, and applying sound professional judgement in relation to risk, eligibility and least-restrictive options. You will maintain accurate and timely records, contribute to multi-disciplinary meetings and ensure that decisions are clearly evidenced. Where appropriate, you will support transitions, hospital discharge planning, legal processes and the use of direct payments or personal budgets.
Applicants must hold a recognised Social Work qualification and current professional registration, with relevant post-qualification experience in adult social care. You should have a good understanding of the Care Act, safeguarding procedures, mental capacity and best-interest decision‑making, as well as experience managing competing priorities in a statutory environment. We are looking for a confident communicator who can build effective relationships, work autonomously and contribute positively to a supportive team. A commitment to equality, dignity, person-centred practice and continuous professional development is essential. The ability to travel within the local area and undertake visits in a range of settings is also required.
