We are seeking a dedicated and compassionate Social Worker to join our Hospital Adults team in Salford. This role is vital in providing high-quality support to adults facing a variety of challenges, including mental health issues, chronic illness, and social isolation. The ideal candidate will be committed to improving the well-being of our patients and their families, working collaboratively with healthcare professionals to ensure comprehensive care.
Key responsibilities will include:
- Conducting assessments of patients’ social, emotional, and environmental needs.
- Developing and implementing care plans in collaboration with multidisciplinary teams.
- Providing emotional support and counseling to patients and their families.
- Advocating for patients' rights and access to necessary services.
- Facilitating discharge planning and coordinating community resources.
To qualify for this position, candidates must possess:
- A recognized Social Work qualification (e.g., Degree in Social Work, DipSW, or equivalent).
- Registration with Social Work England.
- Experience working in a hospital or healthcare setting, preferably with adult populations.
- Strong communication and interpersonal skills, with the ability to work effectively in a team.
- Excellent assessment and analytical skills.
