We are seeking a dedicated and compassionate Qualified Social Worker to join our Adult's Social Work Hospital Team in Salford. This role is crucial in providing high-quality support to adults with complex needs, ensuring they receive the best possible care during their hospital stay and transition back to the community. You will be working collaboratively with healthcare professionals, patients, and their families to assess needs, develop care plans, and facilitate discharge processes.
Key responsibilities include:
- Conducting comprehensive assessments of individuals' social, emotional, and practical needs.
- Developing and implementing person-centered care plans in collaboration with multidisciplinary teams.
- Providing support and guidance to patients and their families throughout the hospital stay.
- Ensuring effective discharge planning and follow-up services to promote successful reintegration into the community.
- Maintaining accurate and up-to-date records in accordance with legal and organizational standards.
The ideal candidate will possess the following qualifications and skills:
- A recognized Social Work qualification (e.g., Degree in Social Work, DipSW, or equivalent).
- Registration with Social Work England.
- Experience working in adult social work, preferably in a hospital or healthcare setting.
- Strong communication and interpersonal skills, with the ability to build rapport with diverse individuals.
- Excellent organizational skills and the ability to manage a varied caseload effectively.
