We are seeking a dedicated and compassionate Social Worker to join our Adults Mental Health team in Rotherham. In this role, you will be responsible for providing high-quality social work services to adults experiencing mental health challenges. You will work collaboratively with individuals, families, and multidisciplinary teams to ensure the best possible outcomes for service users. Your expertise will be crucial in assessing needs, developing care plans, and facilitating access to appropriate resources and support.
Key responsibilities include:
- Conducting comprehensive assessments of individuals’ mental health and social needs.
- Developing and implementing tailored care plans in collaboration with service users and their families.
- Providing emotional support and guidance to individuals and their families.
- Coordinating with healthcare professionals, community services, and other stakeholders to ensure integrated care.
- Monitoring and reviewing progress to adjust care plans as necessary.
- Advocating for the rights and needs of service users within the mental health system.
The ideal candidate will possess a relevant social work qualification (e.g., Degree in Social Work, DipSW, or equivalent) and be registered with Social Work England. You should have a solid understanding of mental health legislation and best practices, along with experience working with adults facing mental health issues. Strong communication, empathy, and problem-solving skills are essential, as well as the ability to work effectively within a team and independently.
