We are seeking a dedicated and compassionate Qualified Social Worker to join our Adult's SW - CAST Team in Richmond & Wandsworth. This role is ideal for an individual who is passionate about making a difference in the lives of adults facing various challenges. You will work collaboratively within a multidisciplinary team to assess, plan, and implement effective support strategies for individuals in need of social care services.

Key responsibilities will include:

  • Conducting comprehensive assessments of individuals' needs and circumstances.
  • Developing and implementing personalized care plans in collaboration with service users and their families.
  • Providing ongoing support and guidance to adults facing mental health issues, learning disabilities, and other complex needs.
  • Working closely with external agencies and stakeholders to ensure a holistic approach to care.
  • Maintaining accurate and up-to-date case records and documentation in accordance with local policies and procedures.

The ideal candidate will possess the following essential qualifications and skills:

  • A recognized Social Work qualification (e.g., Degree in Social Work, DipSW, or equivalent).
  • Registration with Social Work England.
  • Proven experience in adult social work, particularly in community settings.
  • Strong assessment and analytical skills, with the ability to make informed decisions.
  • Excellent communication and interpersonal skills, with a compassionate approach to service users.
