We are seeking a dedicated and compassionate Qualified Social Worker to join our Wellbeing Team in Reading, specializing in adult social work. In this pivotal role, you will work collaboratively with individuals and their families to promote wellbeing, enhance quality of life, and support them through various challenges. Your expertise will be essential in assessing needs, developing care plans, and facilitating access to services that empower adults to lead fulfilling lives.
Key responsibilities include:
- Conducting comprehensive assessments of client needs and circumstances.
- Developing, implementing, and reviewing personalized care plans in collaboration with clients and their support networks.
- Providing ongoing support and guidance to clients, ensuring their voices are heard and respected.
- Advocating for clients to access necessary services and resources, including health, housing, and social supports.
- Maintaining accurate and up-to-date case records and documentation in compliance with organizational policies.
- Collaborating with multidisciplinary teams and external agencies to ensure holistic care delivery.
To be considered for this role, you must hold a recognized social work qualification (e.g., CQSW, DipSW, or equivalent) and be registered with Social Work England. Essential skills include excellent communication and interpersonal abilities, strong analytical and problem-solving skills, and a deep understanding of adult safeguarding and mental health issues. Experience in adult social work and knowledge of local resources and legislation will be highly advantageous.
