We are seeking a qualified Adult Social Worker to join a dedicated Young Adults Team supporting people as they transition from children’s services into adulthood. This is an opportunity to make a meaningful difference by helping young adults achieve greater independence, improve their wellbeing and access the right support at the right time. You will work with individuals aged 18 and above who may have complex care and support needs, including learning disabilities, physical disabilities, autism, mental health needs or behaviours that challenge.
Your responsibilities will include completing strengths-based assessments, preparing and reviewing support plans, arranging appropriate services and safeguarding the rights and wellbeing of the people you support. You will work collaboratively with young adults, families, carers, advocates and partner agencies to identify goals and promote positive outcomes. The role will involve managing a varied caseload, undertaking statutory duties under relevant legislation, contributing to multi-disciplinary meetings and ensuring that records, decisions and financial assessments are accurate and completed within required timescales. You will also support individuals through changes in accommodation, education, employment, healthcare and social care provision.
Applicants must hold a recognised Social Work qualification and current registration with the appropriate professional regulator. You should have experience of adult social care, assessments, safeguarding and care planning, ideally with knowledge of transition work and the needs of young adults. Strong communication, organisation and report-writing skills are essential, alongside the ability to make sound professional judgments and work confidently both independently and as part of a team. We are looking for someone who is compassionate, person-centred and committed to promoting choice, inclusion, dignity and independence. A valid driving licence and the ability to travel to meet service users may be required.
