The Central District Team in Oldham is seeking a qualified Children’s Assistant Practitioner (AP) to join our dynamic team. Your key responsibilities will include:
- Assisting social workers in the assessment and planning processes for children and families.
- Building trusting relationships with children and their families to understand their needs and provide appropriate support.
- Conducting home visits and participating in multi-agency meetings to coordinate services.
- Documenting case notes and maintaining accurate records in accordance with local policies and procedures.
- Providing direct support to children through individual and group activities, including educational and recreational programs.
