As a Social Worker in the Adults - Transformation team, your key responsibilities will include:
- Conducting comprehensive assessments to identify the needs and strengths of individuals.
- Developing and implementing tailored care plans in collaboration with service users and their families.
- Providing ongoing support and guidance to individuals, ensuring they have access to necessary resources and services.
- Working closely with multi-disciplinary teams and external agencies to advocate for the best interests of service users.
- Monitoring and reviewing care plans to ensure effectiveness and making adjustments as needed.
- Maintaining accurate and up-to-date case records in accordance with legal and organizational standards.
To be successful in this role, you must possess the following essential qualifications and skills:
- A recognized Social Work qualification (e.g., BA/MA in Social Work).
- Current registration with Social Work England.
- Proven experience working with adults in a social work capacity.
- Strong assessment and analytical skills with the ability to develop effective care plans.
- Excellent communication and interpersonal skills, with a compassionate approach to service users.
