An exciting opportunity is available for a qualified Adult Social Worker to join a supportive locality team working across a diverse community. You will work with adults who may have care and support needs arising from physical disabilities, learning disabilities, mental health conditions, sensory impairments, frailty or other complex circumstances. The role focuses on promoting independence, wellbeing and choice while ensuring that people receive safe, proportionate and personalised support.
Key responsibilities will include completing strengths-based assessments, reviewing existing care and support plans, identifying eligible needs and arranging appropriate services in line with relevant legislation and local procedures. You will work collaboratively with individuals, families, carers and partner agencies to develop outcome-focused plans, manage risk and safeguard adults where concerns arise. The post also involves contributing to multi-disciplinary meetings, coordinating support packages, making referrals and maintaining accurate, timely records. You will be expected to manage a varied caseload effectively, prioritise competing demands and make sound professional decisions in sometimes complex situations.
Applicants must hold a recognised Social Work qualification and current registration with the appropriate professional regulator. Experience of working with adults in a statutory or community-based setting is desirable, together with knowledge of strengths-based practice, safeguarding, mental capacity, best-interest decision-making and the principles of personalisation. You should have excellent communication and assessment skills, a commitment to equality and person-centred practice, and the ability to build effective relationships with people from a wide range of backgrounds. A valid driving licence and access to transport may be required. In return, you will receive opportunities for professional development, regular supervision and the chance to make a meaningful difference to adults and their families.
