We are seeking a qualified Social Worker to join an Adult Social Care Ageing Well Team in Southend-on-Sea. This is an opportunity to support older adults and their families to live safely, independently and with dignity in their own homes and communities. You will manage a varied caseload, working with individuals who may have physical disabilities, long-term health conditions, mobility needs, frailty, or changing care and support requirements.
Your responsibilities will include completing strengths-based assessments, identifying eligible needs, developing person-centred support plans and reviewing outcomes. You will work collaboratively with individuals, carers, health professionals, care providers and other partner agencies to coordinate effective services and promote wellbeing. The role will involve safeguarding adults, undertaking appropriate risk assessments, contributing to mental capacity and best-interest decisions, and responding professionally to complex or changing circumstances. You will also provide information, advice and guidance on available resources, support planning, personal budgets and relevant community services, while ensuring that statutory duties, recording standards and legal frameworks are consistently met.
Applicants must hold a recognised Social Work qualification and be registered with Social Work England. You should have experience of working with adults, preferably within an ageing well, community, hospital discharge, safeguarding or multidisciplinary setting. We are looking for someone with sound knowledge of the Care Act, Mental Capacity Act and safeguarding procedures, together with strong assessment, communication, organisation and decision-making skills. You will be confident managing competing priorities, maintaining accurate case records and working independently while contributing positively to a supportive team. A compassionate, respectful and solution-focused approach is essential, as is the ability to promote choice, independence and positive outcomes for every person. The post is based in Southend-on-Sea and may require travel to meet people in their homes and community settings.
