We are seeking a dedicated and compassionate Social Worker to join our Adults Mental Health team in Manchester. In this rewarding role, you will be responsible for providing high-quality support to adults experiencing mental health challenges. You will work collaboratively with individuals, families, and multidisciplinary teams to promote mental well-being and improve the quality of life for those in need.
Your key responsibilities will include:
- Conducting comprehensive assessments to identify the needs and strengths of clients.
- Developing and implementing tailored care plans in collaboration with clients and their families.
- Providing ongoing support, guidance, and advocacy to clients navigating mental health services.
- Facilitating access to community resources and services to enhance clients' recovery.
- Maintaining accurate and timely documentation of client interactions and progress.
- Participating in regular team meetings and case reviews to ensure coordinated care.
The ideal candidate will possess the following qualifications and skills:
- A recognized Social Work qualification (Degree in Social Work, DipSW, or equivalent).
- Current registration with Social Work England.
- Experience working with adults in mental health settings.
- Strong assessment and intervention skills, with a person-centered approach.
- Excellent communication and interpersonal skills, with the ability to build rapport with clients.
- Ability to work independently and as part of a multidisciplinary team.
