As a Social Worker in our Adults Integrated Neighbourhood team, your key responsibilities will include:
- Conducting comprehensive assessments of clients' needs and developing appropriate support plans.
- Providing ongoing support and guidance to individuals and their families, ensuring access to necessary resources and services.
- Collaborating with multidisciplinary teams, including healthcare professionals, to deliver integrated care solutions.
- Advocating for clients’ rights and ensuring their voices are heard in decision-making processes.
- Monitoring and reviewing care plans to ensure they meet the evolving needs of clients.
The ideal candidate will possess the following essential qualifications and skills:
- A recognized social work qualification (e.g., Degree in Social Work, DipSW, MA in Social Work).
- Registered with Social Work England.
- Demonstrated experience working with adults in a social work setting.
- Strong communication, interpersonal, and organizational skills.
- Ability to work effectively both independently and as part of a team.
